The Goverment has funded selected aged care providers to offer restorative and rehabilitation programs to older adults.
Transitional Care Programs - Residential and Home Care Based
Transitional care programs are short term and supplied by approved residential aged care and home care providers. Multidisciplinary health care teams will coordinate rehabilitation programs designed to return an older adult to living an independent lifestyle in their own home. There may be input from a physiotherapist, occupational therapist or speech pathologist, involvement depends on the client's assessed need.
Restorative Programs - Home Care Based
Selected HACC providers offer short term restorative programs and/or promoting independence programs designed to assist an older adult regain their independence following an illness, surgery or injury etc. Older adults are provided with a range of home care services including transport to medical and allied health appointments.
Day Therapy Centres - Community Based
Selected aged care providers operate day therapy centres that offer heavily subsidised programs and allied health services to older adults. Services may include:
falls prevention and balance classes
healthy eating support.
